Steps and Qualification for Financial Assistance

to obtain your

                       -Texas Birth Certificates,

                       -Identification Card or

                       -Driver License (No Out of State Birth Cert).

                                 (WE WILL ONLY HELP YOU OBTAIN ONE OF THE ABOVE DOCUMENTS)

Step One:  You must be 17 years of age or older and have a Case Manager or Sponsor who will  

                                 reffer you to our agency.

Step Two: You must be registered for housing, employment, Social Security Benefits or

                        School.

Step Three: By Friday of each week Your Case Manager or Sponsor must email or fax your

                           information ( Name, Address, Phone, Social Security Number, Date of Birth) to                               (mhatcher@1lighthousechurch.com or 817-632-6004

Step Four: Client is responsible for calling the Department of Public Safety or Tarrant Couty

                         Vital Records to find out what is required to obtain their document

Step Five:  When and Where to meet for financial assistance:

                         Texas Identification Card or Driver License

                         On Tuesday at 10:30 AM, with all the required documents, you must meet at the 

                         Texas Public Department Safety
